In order to ensure safe and effective massage therapy treatments,
we collect your personal and health information on a confidential
health history form. After each massage, your therapist makse
treatment notes. This health information is your personal
property, and it (as well as any discussions during your treatment)
stays private between you and your therapist.
1. To treat and care for you
2. To contact you about appointments or treatment options
3. To consult with supervisors about your care (without revealing
your identity)
4. To plan and administer internal operations
5. To receive payment for your care from a third party
6. To comply with legal and regulatory requirements
If we need your information for some other purpose not listed
here, we will notify you and get your permission to use it.
1. We only collect information that is necessary to treat
you effectively
2. We continually make sure your information is up to date
3. We keep your information private and under lock and key
4. We give you access to your health records if you wish to
see them
5. You can make corrections to your records at any time
6. You may get a copy of your file if you wish, with a small
fee to cover copying costs
7. We get your permission to share your health records with
other health care providers
8. We keep your records for 10 years from your last visit,
and then destroy them securely
