Privacy Policy

I take your privacy seriously. I do not share your personal or health information with anyone, except: when you ask me to do so; to care for you effectively; as required by law; or in order to collect payment (e.g. from your insurance company).

Here is a full explanation of the privacy policy of Body Poets Massage Therapy (Ellen Symons, RMT):

In order to ensure safe and effective massage therapy treatments, I collect your personal and health information on a confidential health history form. After each massage, I make treatment notes. This health information is your personal property, and it (as well as any discussions during your treatment) stays private between you and your therapist.

Some of the reasons I collect your personal and health information are:

  1. To treat and care for you
  2. To contact you about appointments or treatment options
  3. To receive payment for your care from an insurance company or workers’ compensation board
  4. To plan and administer internal operations
  5. To consult with supervisors about your care
  6. To comply with legal and regulatory requirements

If I need your information for some other purpose not listed here, I will notify you and get your permission to use it.

I take your privacy seriously, and I make sure your personal and health information stays private. My privacy policy is modeled on Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA) and Ontario’s Personal Health Information Protection Act (PHIPA).

Personal information is anything that could identify you (this does not include your business address or business phone number); personal health information is information about the state of your health and your treatments.

Here’s what I do at Body Poets Massage Therapy to protect your personal and health information:

  1. I only collect personal and health information that is necessary to treat you effectively.
  2. I continually make sure your information is up to date.
  3. I keep your information private and under lock and key. Only your therapist (Ellen Symons, RMT) has access to it.
  4. I give you access to your health records if you wish to see them.
  5. You can make corrections to your records at any time.
  6. You may get a copy of your file if you wish. I will charge you a small fee to cover the cost of photocopies.
  7. I won’t share your health records with anyone unless I have your written permission to do so, or if the law requires me to do so.
  8. I keep your records for 10 years from your last appointment, and then I destroy them securely.

The law says I do not need your permission to share your records with:

a) others involved in your health care (e.g. doctor, chiropractor)
b) in order to receive payment (e.g. from an insurance company)
c) as required by law.

However, it is my policy to get your permission before sharing your personal or health information with any other health care professional.
